Insulin-Using Commercial Motor Vehicle Drivers

This document represents the outcome of a synthesis sponsored by the Office of Motor Carriers to review the current medical standard covering commercial drivers with diabetes mellitus.  The Standard (49 C.F.R. section 391.41 (b) (3)) permits qualification of individuals to drive a commercial motor vehicle if that person has no established medical history or clinical diagnosis of diabetes mellitus currently requiring insulin for control.

A literature review of accidents involving diabetic drivers was conducted along with a review of the current state laws regulating insulin-using CMV drivers.  The effectiveness of current diabetes treatment was presented.  A risk assessment was developed and applied to the information available concerning insulin-using drivers.  Public comments submitted to the FHWA in response to the Notice of Proposed Rulemaking on the qualification of drivers with insulin-treated diabetes for CMV operation were tallied (125) and summarized.  The role of a medical advisory board in individual petitions, development of regulations and the establishment of monitoring procedures were discussed.
